Possible online services disruption due to Internet related outage

A worldwide technology outage is causing disruption to some State of Illinois online systems.  We are aware of this issue and are diligently working on restoration.

Report Tax Fraud and Related Violations Online

To report tax fraud, choose one of the following:

  • Individual Income Tax Fraud - Use this form to report the following:
    • Dependent Claims (fraudulent child claims on tax returns)
    • Individual Non-filer/Fraudulent Filer Reporting (to report an individual who is not filing their taxes, who is underreporting their income, who is falsifying their records, etc.)
    • Tax Preparer Fraud or Violations
    • Self-Employment Complaints (false self-employment, false business income on return)
    • Questions on Individual Income Tax Letters Received from the Department
      Note: Unless you feel that you have received a correspondence from the Department due to Fraud, the Department advises that questions about correspondences are handled more timely when contacting the Department in the manner described on the correspondence rather than filing a Fraud Report.

Note: To report individual identity theft, visit the identity theft page and fill out Form IL-425.

  • Audit Level Tax Fraud - Use this form to report the following:
    • Non-Profit Scams (a business is operating as a charity for tax purposes but does not qualify as a tax exempt organization)
    • Cash Only Complaints (a business only pays employees in cash to avoid reporting their income)
    • No WIT Employer/Unclaimed Employees (a business does not file withholding for their employees, staff work “under the table”, etc.)
    • Part-Year/Non-resident Complaints
  • Collections, Debt, and Lien Fraud - Use this form to report the following:
    • No Registration (business is not registered, illegal operation, certificate issues, etc.)
    • Debt/Lien Complaints
  • MyTax Illinois Issues - Use this form to report the following:
    • MyTax Illinois Issues Related to Tax Fraud
  • Registration Issues - Use this form to report the following:
    • Registration Issues Related to Tax Fraud
  • Charitable Games Tax Fraud - Use this form to report the following:
    • Bingo and Charitable Games Complaints
  • Property Tax Fraud - Because property taxes are collected at the local level, Property Tax Fraud should be reported to the county rather than the state. Contact your county official for more information.
  • Business Income Tax Fraud - Use this form to report the following:
    • Business Non-filer/Fraudulent Filer Reporting (to report a business who is not filing their taxes, who is underreporting their income, who is falsifying their records, etc.)
    • Withholding Non-filer/Fraudulent Filer Reporting (to report an employer has not filed but employees are claiming withholding, etc.)
    • Accounting Discrepancy/Accountant Complaints
  • Sales and Use Tax, Cigarette Tax, Tobacco Products Tax, Motor Fuel Tax, and “Other” Fraud - Use this form to report the following:
    • Reporting Issues (business not reporting sales made in cash, hiding sales, charging wrong amount, etc.)
    • Cigarette Tax Violations (selling cigarettes with other state’s stamps or without stamps, selling individual cigarettes, sale or delivery of contraband cigarettes, etc.)
    • Tobacco Products Tax Violations (unlawful distribution, sales, or delivery and other complaints involving other tobacco products, including electronic cigarettes)
    • IFTA and Dyed Diesel Violations
    • Unlicensed Auto Dealer
    • Other Tax Complaints
