I have lost or misplaced my refund check, how do I go about getting a new one?

To request a replacement refund check, you may call the Comptroller's Office at 1 800-877-8078 or write to:

Include your name, address, Social Security number (SSN) or Federal employer identification number(FEIN), check number, and refund amount (if known).

To get a replacement for a check that is uncashed and more than one year old (expired), send the uncashed check to the address listed above and request a new one in writing.
