EFT-1, Authorization Agreement for Electronic Funds Transfer (Note: Limited to certain tax types)

Complete Form EFT-1 ONLY if you 1) want to pay amounts owed to the Illinois Department of Revenue using pay-by-phone debit or cigarette tax stamp purchase debit; or 2) are changing previously submitted information about one of the methods of payment listed above.

Note: For most taxes, electronic payments can be made easily using MyTax Illinois available at mytax.illinois.gov. Do not complete Form EFT-1 if you pay using MyTax Illinois.

You cannot use Form EFT-1 to register with the Illinois Department of Revenue for authorization to make ACH Credit payments. You must use MyTax Illinois to register for ACH Credit authorization. At the MyTax Illinois login screen, select "Request ACH Credit Banking Instructions" in the Additional Services panel. You will be contacted by the department after we process your ACH Credit authorization request. We will reject all attempts to use Form EFT-1 for this purpose. Using MyTax Illinois is the only method to submit an ACH credit request for authorization.

How do I change the information I have previously submitted on Form EFT-1?

You must complete a new Form EFT-1, Authorization Agreement for Electronic Funds Transfer. Make sure you mark the box indicating you are submitting the form to report changes to previously submitted information and write the date the change is effective in Step 1. For quickest service, you may fax your completed form to us at 217 524-8282.

Any changes related to business or contact information also must be reported to our Central Registration Division. MyTax Illinois, our online account management program, allows you to make these changes electronically. You also may contact our Central Registration Division by phone at 217 785-3707 or via email by using the "Questions, Comments, or Request" link below. Please include your Illinois account ID or license number, business name, and a detailed description of the maintenance requested.

Instructions for approved, first time users of the electronic funds transfer phone system:

Enter four zeros (0000) into the system following the EFT-10 instructions. The system will then prompt you to enter your own four-digit password.
