How to complete PDF forms
Important information about PDF forms
All PDF forms on this website must be downloaded before entering any information. Forms filled out inside a web browser may not save correctly, and information can be lost. Downloading the form before use ensures full function and proper saving.
Some PDF forms may open with an instructional first page or show a “Please wait…” message. These messages appear when a web browser cannot display advanced PDF features such as expanding fields, tables that add rows, buttons, and signature tools. Full support for these form features requires PDF forms to be opened specifically in Adobe Acrobat.
Instructions for downloading and completing PDF forms
- Download the PDF file
- Right‑click the PDF link and select “Save link as…”
- Save the file to the Desktop, the Downloads folder, or any other preferred location
- Do not fill out the form in the browser window
- Open the file in Adobe Acrobat
- Open Adobe Acrobat Reader or Adobe Acrobat Pro
- Go to File > Open
- Select the downloaded PDF file
- Complete the form in Adobe Acrobat and Save
- All fields, expanding areas, buttons, and signature tools will work correctly when opened in Adobe Acrobat, and the form will save as expected.
Why downloading is required
Web browsers do not support many interactive PDF features. These limits can prevent forms from displaying or saving correctly. Using Adobe Acrobat provides full support and reduces the risk of lost information.
