Telecommunications Excise Tax Forms

Effective July 1, 2025, the state Telecommunications Excise Tax rate includes the state 9-8-8 Suicide and Crisis Lifeline surcharge. See FY 2025-26, Telecommunications Excise Tax Rate Change, Effective July 1, 2025.

Eligible forms may be filed electronically using MyTax Illinois, a FREE online account management program.
