Attention Firefox Users

Firefox uses a built-in viewer to present PDF documents by default.

This viewer does not allow users to fill in form fields. To allow fill-in forms, follow these instructions to re-enable the Adobe Acrobat Reader.

Attention Chrome Users

Chrome uses a built-in viewer to present PDF documents by default.

Some advanced features used in our PDF forms may not work with this built-in viewer. If you experience difficulty with a fill-in form, you can download the form for viewing with the Adobe Acrobat Reader by clicking the save button that appears in the lower right when you hover your mouse in that area.

You should file Form CPP-1, Installment Payment Plan Request, if you have tax delinquencies that you cannot pay in full because of a financial hardship and you would like to enter into a payment installment plan with us.

A payment installment plan is an agreement between you and the Illinois Department of Revenue to pay your tax delinquencies using a monthly payment plan. Your monthly payment amount and the length of time that you have to pay is based on your financial condition.

Note: You must complete a Form EG-13-I, Financial and Other Information Statement for Individuals, or a Form EG-13-B, Financial Statement for Businesses, if the payment agreement amount that you are applying for, including penalty and interest, is over $5,000.
