Beginning with the 2005 registration period, the US DOT number will be a required field. Step 1, Line 3 of Form MFUT-12, Application for Motor Fuel Use Tax IFTA License and Decals, must now be completed. Failure to complete this line will result in a delay or rejection of processing your application/renewal.
